Thrive Testing Solutions, LLC

1. Acceptance of Terms
By scheduling or receiving services from Thrive Testing Solutions, LLC (“Company,” “we,” “our,” or “us”), you (“Client,” “Patient,” or “you”) acknowledge that you have read, understood, and agree to these Terms and Conditions.
2. Services Provided
Thrive Testing Solutions, LLC provides mobile specimen collection services, including but not limited to:
- Saliva specimen collection
- Urine specimen collection
- Other approved specimen collection services as authorized by a licensed healthcare provider or laboratory
Our services are limited to specimen collection. We do not diagnose medical conditions, interpret laboratory results, prescribe medications, or provide medical treatment.
3. Physician Orders
A valid physician, healthcare provider, laboratory, or authorized client order may be required before specimen collection. The Company reserves the right to refuse collection if required documentation is incomplete or invalid.
4. Appointments
Appointments are scheduled based on availability. Clients agree to:
- Provide accurate contact and location information.
- Ensure someone 18 years or older is present if required.
- Have identification and required paperwork available.
- Agree to a safe, clean, and accessible environment for the specimen collection.
The Company reserves the right to decline or terminate services if the environment is deemed unsafe.
5. Fees and Payment
Payment is due at the time of service unless prior billing arrangements have been made. Accepted payment methods include:
- Credit or debit cards
- Electronic payment
- Cash
Additional fees apply for:
- Travel outside the standard service area
- After-hours appointments
- Weekend or holiday appointments
- Multiple collection attempts
- Missed appointments or cancellations
All fees are non-refundable once services have been completed.
6. Mobile Transit & Travel Fee Policy
A travel fee applies to all mobile specimen collection appointments to cover vehicle wear, fuel, and technician transit time.
- Standard Travel Rate: $.75 per mile.
- How It Is Calculated: Mileage is calculated for a round trip starting from our central zip code (06850) to your requested service location using standard Google Maps routing.
- Non-Refundable Policy: Once our technician leaves our facility and transit begins, the travel fee is 100% non-refundable, even if the sample collection cannot be completed due to patient absence, or an incorrect address.
7. Cancellation Policy
Clients who are unavailable upon arrival will have to pay for a new test. Cancellations will not be refunded, but you may reschedule your appointment as long as it is at least 48 hours before the appointment. If it is not at least 48 hours before your appointment, you will be charged a $50.00 reschedule fee.
8. Refusal of Service
The Company reserves the right to refuse or discontinue services when:
- A required laboratory order is unavailable.
- The patient refuses the procedure.
- The collection cannot be safely completed.
- The patient is abusive, threatening, or disruptive.
- The location presents safety concerns.
- The specimen cannot be collected according to professional standards.
9. Specimen Integrity
We follow established specimen collection protocols to maintain specimen integrity. However, we are not responsible for:
- Laboratory testing errors.
- Delays caused by shipping or courier services.
- Laboratory rejection due to improper patient preparation.
- Hemolyzed or otherwise compromised specimens resulting from patient-specific factors beyond our control.
- Delays caused by weather, accidents, or other unforeseen circumstances.
10. Patient Responsibilities
Clients agree to:
- Provide truthful health and identification information.
- Follow any fasting or preparation instructions provided by the ordering provider or laboratory.
- Notify us of allergies, bleeding disorders, or medications that may affect specimen collection.
- Follow post-procedure instructions.
11. Privacy and Confidentiality
We are committed to protecting patient privacy. Protected health information will be handled in accordance with applicable federal and state privacy laws, including HIPAA where applicable. Patient information will only be disclosed as permitted by law or with proper authorization.
12. Limitation of Liability
To the fullest extent permitted by law, Thrive Testing Solutions, LLC shall not be liable for indirect, incidental, special, or consequential damages arising from:
- Delayed laboratory results
- Laboratory errors
- Missed diagnoses
- Medical decisions made by healthcare providers
- Events beyond our reasonable control
Our liability, if any, shall be limited to the amount paid for the specimen collection service. Nothing in these Terms limits liability where such limitation is prohibited by applicable law.
13. Medical Emergencies
Our personnel do not provide emergency medical services. If you experience chest pain, difficulty breathing, loss of consciousness, severe bleeding, or any other medical emergency, call 911 immediately or seek emergency medical care.
14. Consent to Collection
By receiving services, you voluntarily consent to specimen collection and acknowledge that:
- The procedure has been explained.
- You have had an opportunity to ask questions.
- You understand there are minor risks, including bruising, bleeding, dizziness, discomfort, infection, or unsuccessful collection.
15. Independent Service
Unless otherwise agreed in writing, Thrive Testing Solutions, LLC operates as an independent specimen collection service and is not an employee or agent of any physician, hospital, or laboratory.
16. Compliance with Laws
The Company complies with applicable federal, state, and local laws governing specimen collection services.
17. Intellectual Property
All Company materials, forms, branding, logos, and website content remain the property of Thrive Testing Solutions, LLC and may not be copied or reproduced without written permission.
18. Governing Law
These Terms and Conditions shall be governed by the laws of the State of Connecticut, without regard to conflict of law principles.
19. Severability
If any provision of these Terms is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
20. Changes to These Terms
The Company reserves the right to update these Terms and Conditions at any time. Updated versions become effective upon posting on our website or otherwise providing notice to clients.
